 Applicant Status

Residents of Barbados referred by Medical Practitioner

1. Non-Residents of Barbados

2. Barbadian Residents who independently wish to take up treatment overseas

Documentary Evidence Required

1. Form FC(Not for Imports)

2. Letter from the applicants's local physician stamped by the Chief Medical Officer (MOH in Barbados)                                             

1. Form FC(Not for Imports)

2. Tax Clearance Certificate (Non-residents only)

3. Statement of related expenses from medical institution or and a breakdown of how funds are to be otherwise utilized.

Steps to Take

Carry out transaction at commercial bank.

Collect FC Form from the Central Bank. Proceed to the Inland Revenue Department to acquire Tax Clearance Certificate. Bring all required documentation to the Central Bank for processing.
